How would you go about this? I'd like my visitor to provide their name, email, full mailing address (for a physical freebie) but then would like an optional few questions as part of a brief survey.
I currently have an Aweber subscription. Is there anyway I could have all this data imported? Or should I just have contact info imported? But then can I somehow have the form email the rest to a specific mailbox?
Does anyone know of a script that can do this?
